WVHA HealthCard Program
What are the qualifications?
You must live in West Volusia for at least three months (one month for homeless). Your income must be below, equal or up to 150% of the Federal Poverty Level Guidelines for your family size.

$0 Clinic visits
$0 Prescription medicines
$50 Emergency room visits
$10 Specialist visits
$50 Hospital services
$10 Imaging services
$25 Urgent care
How Can I Get a WVHA HealthCard?
You must complete an application and provide the following documents:
- Proof of residency Two (2) forms of identification – One (1) must be a picture ID
- Proof of Income/Assets
- Proof of Medicaid Application or Medicaid Application Denial Letter
- Proof of Affordable Care Act (Healthcare.gov) Application
Required HealthCard Forms
Form One: HealthCard Application Form
All members of the Household may apply through the same application. Please indicate all applicants in Section 2 “Members of the Household.”
Form Two: HealthCard Assessment Form
Please complete this form in its entirety. This form must be completed by all applicants over 18, including legal spouses who are not applying.
If you would like a copy of your entries on either form, print a copy or to save a copy to your computer, click Print and then save as a PDF BEFORE YOU PUSH SUBMIT.
Push submit to have your completed forms emailed to The House Next Door. They will call you to schedule an appointment.
